National One-Rehabilitation Framework (One-Rehab)

One-Rehab is a national rehabilitation outcomes and care coordination framework that enables consistent measurement, benchmarking and tracking of patients’ rehabilitation journeys across care settings.
One-Rehab harmonises rehabilitation assessment, care planning and outcome tracking across acute, post-acute and community-based settings. The framework aims to unify rehabilitation care across Singapore through a common IT system that supports shared data and outcome tracking, enabling patients to receive the right rehabilitation care at the right time and place – whether in hospital or within the community.
The initiative is led by the Ministry of Health, in partnership with MOHT, Agency for Integrated Care (AIC), Synapxe, Singapore Institute of Technology (SIT), public healthcare institutions (PHIs), and community rehabilitation providers.
MOHT contributed to the design and implementation of the framework, including cross-provider coordination, alignment of care pathways, and development of the shared IT and data systems needed to support integrated rehabilitation care.
Aim Statement
To improve the rehabilitation journey from acute to community settings by harmonising care pathways, building robust partnerships between acute and community care providers, enabling seamless provider collaboration and sustainable care, and supporting shared outcomes tracking across the system.
Why It Matters
Demand for rehabilitation services is rising with Singapore’s ageing population. Acute-centric rehabilitation models limit accessibility and sustainability. One-Rehab is a MOH-led initiative to support the shift in care to community-anchored rehabilitation, in ways that can increase care accessibility for patients. Through a minimum data set shared among a patient’s care teams, One-Rehab synchronises communication among providers, harmonises rehab care planning, and creates IT-enabled visibility of patient journeys.

Key Results To Date
Led the platform design and implementation in collaboration with Synapxe and other partners. During the pre-pilot phase, MOHT led the engagement with providers to understand their workflows and requirements, ensuring the system was designed to align with operational realities. Following the system’s launch, MOHT continues to drive iterative improvements and oversees Synapxe’s operational delivery and system maintenance.
Enabled rehabilitation teams – including clinicians, physiotherapists, occupational therapists and speech therapists – across the three healthcare clusters and 18 community providers to formulate care based on the harmonised, structured information captured through the One-Rehab Care Plan.
Established clear data specifications across providers to allow integration of data across multiple care settings.
One-Rehab Platform IT system integrated across multiple institutions since August 2022 with active clinician usage across clusters.
Up to 17,000 patients supported annually, with >288,000 patients onboarded.
Achieved strong scale, broad institutional uptake, and sustained system improvements since launch.
Evaluation findings demonstrate clinical safety and effectiveness with readiness for mainstreaming and transition to national ownership (MOH/Synapxe) in FY2026.
Rehabilitation outcomes comparable across Specialist Outpatient Clinics, polyclinics, and community settings in evaluated conditions.
