Our data reported significantly shorter LOS, non-inferior clinical and functional outcomes, superior experiential outcomes (from both patient and caregiver perspectives) and significantly lower caregiver stress levels at 90 days post discharge. In addition, overseas studies also demonstrated other positive benefits such as reduced rates of iatrogenic complications (e.g. Incident delirium and nursing home placement rates).
The biggest benefits for patients are faster rates of recovery in their own familiar home environment, in the presence of loved ones, and avoidance of hospitalisation-related complications especially for frail elderly with dementia. The care team is able to assess patient in their home environment to more accurately determine care needs and personalise treatment goals to their lifestyle. Patients and caregivers report feeling more satisfied with care experience under MIC@Home. They are more empowered with self-management & are actively involved in decision making.
