Towards Future Healthcare
Commemorating MOHT's 3rd Anniversary

Message from the Executive Director
MOHT has come a long way in a relatively short time. When we started in 2018, we were tasked to look closely at the fundamental challenges facing our healthcare system, and find solutions to these challenges leveraging advances in data science, digital technologies and medicine.
We forged partnerships with both public and private organisations while maintaining strong links to the Ministry of Health and other government bodies.
With our partners, we launched initiatives such as the Integrated General Hospital model of care, the Primary-Tech Enhanced Care system for select chronic diseases, toolkits for communities and government bodies to promote healthier living, and online resources for mental wellness. Most recently we deployed many solutions in the national fight against COVID-19. Through these initiatives, we also helped build the enablers necessary for at scale adoption.
I would like to thank everyone who has been part of the MOHT journey so far, and we look forward to working together to make a transformative impact on our healthcare system.

Key Milestones
2018 - 2021

2018
One bed, one care team
MOHT partnered Alexandra Hospital to roll out a new model of care, where patients are cared for by a single care team with minimal transfers during their inpatient stay, and their post discharge care is well integrated with the community.
2019
Giving greater ownership to communities
MOHT's HPT was highlighted as one of 11 contributions to the HealthySG Taskforce recommendations, which aimed to transform Singapore’s health promotion landcape, with the infusion and integration of health in various aspects of our lives and the environment.

2019 - 2020
Bringing care for chronic conditions closer to home
MOHT piloted and launched Primary-Tech Enhanced Care (Hypertension), a telehealth programme for patients with high blood pressure. The system is being rolled out across all three polyclinic clusters.
2020
mindline.sg - a one-stop support portal
MOHT led the development of mindline.sg, a portal to enable self-care and management of whole-of-person wellbeing during the COVID-19 pandemic and beyond.

2020 - 2021
Fighting COVID-19
Rapid solutioning to combat the pandemic
At the height of the COVID-19 pandemic, MOHT teams quickly refocused efforts to develop solutions to aid healthcare teams and the public. Solutions included an easy-to-use Symptom Checker, design sprints to ideate community-led initiatives, and digital tools to enhance patient workflows for care teams.
