The Primary Tech-Enhanced Care (PTEC) Home HbA1c Testing (HAT) Pilot jointly conducted by MOHT and NHGP aims to help patients with Diabetes Mellitus to better monitor their blood glucose levels. The HbA1c test measures your average blood glucose levels over the past 2 – 3 months, and readings are automatically shared with your care team. Through tele consultations with your care team and the bilingual (English and Chinese) PTEC Chatbot, you will be able to better manage your Diabetes while reducing visits to the Polyclinics.
